Who We Are: Purposeful, Progressive, Dynamic

Spiras Health is an in-home healthcare provider taking an innovative approach in the delivery of personalized clinical care to patients with complex needs. We collaborate with patient’s physicians, health plan, discharge planners, and other ancillary providers to help improve quality of life, improve compliance, and reduce avoidable emergency room and hospital visits. We are privileged to serve patients who are living with several chronic and complex conditions. Our vision is to deliver patient care in the comfort of their homes while reducing their fear, uncertainty, and anxiety about their health. We use technology to connect clinicians in the home with additional expertise and have developed a team structure to ensure we do so consistently. Our culture is anchored on a promise of full accountability and integrity in everything we do.

How We Serve

The Enrollment Specialist I (ES I) is a fast-paced, telephonic patient recruitment role with no face-to-face patient contact but outreaches to potential program participants and connects them as a patient with Spiras Health. The ES I reaches out to patients telephonically that may be a fit for the Spiras Health Solution as identified through the Spiras Analytics predictive model. This role will be accountable to influence the potential participant to engage with Spiras by scheduling an in-home clinical assessment when they are deemed a good fit for the program. The ES I fills a crucial initial step by making potential participants aware of the opportunity and obtaining permission for a member of our clinical team to meet with them in their home.
