How We Serve

This position calls for a  high performing Licensed Practical Nurse that will become part of a Clinical Care Team, working in partnership with Nurse Practitioners. Together the team will manage the overall health of a defined population of patients in collaboration with the patient’s primary care and specialist physicians. You will serve your community which includes daily travel to patients’ homes. Meeting with patients in their home affords you the opportunity to develop deep and rich relationships. 

This individual will understand and be comfortable with mobile technology and have a deep commitment to exceptional patient care. A typical day may include home patient clinical assessments, telephonic assessments of patients, providing patient education, maintaining patient compliance to a treatment plan, connection to community resources, conducting routine diagnostic tests, and charting assessments in Spiras systems. This is a personally and professionally rewarding position and it allows the colleague an opportunity to provide health care services at the top of their training. Each day is a mixture of working from your home office and seeing patients in the community. This results in some flexibility in work hours with the constant focus being patient centered care. 

Duties and Responsibilities

  • Conducts detailed patient assessment, including vital signs, monitors for changes in status, reports findings, contribute to and implements care plans, collaborates with Nurse Practitioner, provides patient education
  • Helps patient accomplish treatment plan goals and identifies and seeks to resolve barriers to care
  • Evaluates effects of treatment plan by observing, noting, and evaluating patient’s progress and recommending adjustments and modifications
  • Communicates with physicians, nurses, social workers, family, caregivers, and other health care workers
  • Recommends assistive equipment and outpatient or home health follow-up programs
  • Accurate and timely documentation of patient care and services per policy
  • Always adheres to hygiene and safety standards (sanitizing surfaces, equipment, hands, etc.)
  • Fosters a supportive and compassionate environment to care for patients and their families
  • Consistently adhere to documented policies, procedures, and standards
  • Other duties as assigned
  • You will be joining a market team. You may be asked to cover any patient within this market. However, your home territory will typically be a 60 mile radius from your listed home address. Standard scheduling procedures have been designed to limit drive time and increase our ability to provide patient care. 
  • Additional travel required 

 Qualifications 

  • Successful completion of LPN licensing program
  • Minimum 2+ years of experience as an LPN
  • Excellent knowledge of nursing care methods and procedures
  • Excellent knowledge of emergency care
  • Valid and active state nursing license
  • Reliable and insured personal vehicle
  • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification 
  • Experience with cardiac or pulmonology specialties
  • Experience with advance directives and palliative care
  • Experience with clinically complex geriatric patients

 Preferred Qualifications

  • Bilingual (English / Spanish)
  • Home Health experience

  • Strong and concise verbal and written communicator
  • Able to rapidly identify and gather required information to assess and care for 
  • patients
  • Comfortable working with patients that may have difficulty hearing, struggle 
  • with memory issues, or have a lower health care literacy level
  • Desire to collaborate with others
  • Ability to think quickly and provide information with confidence and clarity
  • Ability to manage competing priorities, organize work efficiently, and meet deadlines
  • Perseverance and self-management to achieve defined objectives
  • Confident working with EMR platform and mobile applications
  • Strong knowledge of chronic medical conditions are the keys to success in this role.

  • This job operates in a remote professional environment free from noise and distraction. This role routinely uses standard office equipment such as computers and phones.
